Job summary

Family Services Windsor-Essex (FSWE) is seeking a bilingual Community Programs Social Worker to support youth, adults and seniors across multiple community programs. You will assess referrals, design individualized plans, and provide in-home as well as community support to help clients navigate health, housing and financial challenges.

The role emphasizes advocacy, collaboration with partners, and empowering clients to achieve independence while maintaining dignity and self-determination.

Qualifications

• Bachelor or Master of Social Work is required.
• Fluency in English and French (written and verbal) is required.
• OCSWSSW registration or eligibility is preferred.
• Experience with case management for vulnerable groups is required.

Responsibilities

• Receive referrals and assess client eligibility, needs, and goals.
• Develop, implement, and monitor individualized service plans.
• Provide direct community and in-home support to clients.
• Support clients in achieving independent living and using resources.
• Help navigate healthcare, housing, income, legal, and other systems.
• Advocate and educate clients about rights and options.
• Collaborate with partners to coordinate supports and outcomes.
• Identify referrals to appropriate community resources.
• Maintain confidential documentation and complete reports.
• Participate in case reviews and inter-disciplinary planning.
• Keep knowledge of resources, legislation, and best practices up to date.
