Qualifications

  • 2–4 years of social work experience, with preference for children
  • Understanding of social services system is preferred
  • Determine eligibility for services and assist referrals for local/out-of-town placements
  • Complete CSN applications and reviews as required
  • Prepare budgets and transition planning
  • Coordinate placement and respite supports with partners
  • Attend Plan of Care meetings for residential placements
  • Endorse CSN-related letters to clinical teams
  • Produce required ministry reports and attend meetings
  • Participate in supervision and seek consultation as needed

Job description

Reporting to the manager, The Mental Health Social Worker I is responsible for coordinating service applications for children and youth with complex special needs who may require placement or services outside of Windsor/Essex County. The Social Worker will attend community case resolution meetings or community case conferences.This position involves working as a member of an inter-professional team offering comprehensive support to children/youth in Windsor/Essex with highly complex mental health and/or developmental needs and their families. The Mental Health Social Worker I assumes the role for Complex Special Needs (CSN). In this role, she/he is responsible for, among other things, assisting in the referral process for local and out of town placements and coordinating and completing complex special needs applications . The Mental Health SW I is responsible for delivery on care consistent with the vision and values of the hospital and the standards of relevant professional organizations.The Mental Health Social Worker I complies with the Occupational Health and Safety standards for the hospital and is proactive in promoting a safe work environment.

Qualifications

2 – 4 years’ experience working in a social work capacity with preference given to children’s – Preferred

Good understanding of social services system - Preferred

Determination of eligibility for services and assist in the referral process for local and out of town placements

Completion of CSN application and reviews as required

Budget preparation

Transition planning

Coordinate placement and respite supports; liaising with partners

Attend Plan of Care meetings for residential treatment placements; review set goals

Endorse letters to CPRI clinical team for CSN

Ministry Reports as required

Attend scheduled Departmental and Service/Program meetings

Fulfills other duties as assigned to ensure that services are being provided in an efficient and effective manner.

Participates in staff in-service training and consultation with other staff

Responsible for the completion of relevant reports in accordance with established standards (e.g., progress notes, assessment reports, etc.)

To participate in regularly scheduled supervision and seek out supervision and consultation as needed to provide effective services to assigned children/youth and families.

To promote constructive relationships amongst service providers and children, adolescents and families

French Language proficiency is an asset

Education and/or Certification

Bachelors of Social Work Degree (BSW) - Required

Current Certification of Registration issued through the Ontario College of Social Workers & Social Service Workers - Required
