Job summary

Match Group Recruitment is seeking a Registered Practical Nurse in Ontario to lead wellness services for a retirement residence. You will collaborate with the Director of Wellness to uphold standards, assess residents, and develop care plans.

The role involves staffing oversight, continence program management, medication administration supervision, onboarding staff, and building hospital partnerships. A current RPN license in good standing and at least two years in geriatrics or rehab are

Qualifications

• Current registration in good standing with the College of Nurses in Ontario as a Registered Practical Nurse.
• Minimum 2 years’ experience in geriatrics, rehabilitation and/or retirement living.

Responsibilities

• Collaborate with the Director of Wellness to promote and ensure standards of practice; complete stable resident re-assessments and initial assessments when DOW absent, develop service plans and coordinate family care conferences.
• Assess resident care needs; develop plans of care and determine corresponding care fees and staffing requirements.
• Coordinate staffing levels with occupancy, resident acuity and services offered.
• Oversee the Continence Care Program; complete assessments, orders, sizing, and service transitions.
• Oversee and supervise the medication administration program; administer medications, complete audits, monitor controls, train staff, and safeguard drugs and narcotics.
• Manage Wellness Team onboarding and education requirements.
• Develop professional relationships with hospital staff and participate in marketing outreach for wellness services and aging in place philosophy.
• Perform other duties as assigned

• Collaborating with the Director of Wellness to promote and ensure standards of practice are achieved and evaluated; completing all stable resident Re-assessments in conjunction with Nursing team, in absence of DOW complete Initial assessments, developing service plans and arranging resident/family care conferences as required to ensure all data is valid and up to date.
• Ensuring resident care needs are assessed; developing plan of care and determining corresponding care fees and staffing requirements.
• Collaborating with the Director of Wellness to schedule and adjust staffing levels to ensure consistency with the residence’s occupancy levels, the acuity of the residents and the services being offered.
• Provide oversight and management of the Continence Care Program; completing assessments, orders, sizing, and service transitions.
• Overseeing and supervising the medication administration program including administering of medications, completing audits, monitor control, training of new team members and safekeeping of drugs and narcotics.
• Manage Wellness Team Onboarding and education requirements
• Developing professional and collaborative relationships with hospital staff, participate in marketing outreach such as presentations on wellness services and aging in place philosophy
• Performing other duties as assigned
