Job summary

Chartwell Erinview Retirement Residence seeks a Resident Wellness Coordinator to support the Health and Wellness Manager in delivering personalized nursing care, coordinating wellness services, and guiding front-line staff.

You will oversee care plans, liaise with physicians and families, ensure documentation and regulatory compliance, and foster resident-centered care in a dynamic retirement living environment.

Qualifications

• Minimum of two years’ experience in retirement living or related healthcare.
• Experience with resident assessments, care planning, and medication administration.
• Experience supervising front-line care staff is an asset.
• Strong documentation and administrative skills.

Responsibilities

• Assess, plan, implement, and evaluate individualized resident care plans.
• Provide direct nursing care and wellness support to residents as required.
• Administer medications and monitor changes in condition per policies.
• Update care plans, wellness documentation, and health records as required.
• Communicate changes in resident condition to physicians and families.
• Liaise with physicians, pharmacists, hospitals, and external providers.
• Participate in admissions, transfers, and discharge planning.
• Support physicians during visits and ensure follow-up actions.
• Encourage participation in therapy, wellness, and recreation programs.
