Job summary

Métis Nation of Ontario (MNO) is seeking an Aging at Home Coordinator to support Citizens 55+ with in‑home and community-based services. The role emphasizes keeping clients independent, well, and connected with culturally relevant supports.

The position involves coordinating services, managing documentation, and partnering with service providers, volunteers, and MNO teams to maximize wellness and quality of life for seniors.

Qualifications

• College diploma in social services or related field.
• 1–3 years of relevant work experience.
• Experience providing client services and working with seniors 55+.

Responsibilities

• Deliver Aging at Home services in line with MNO principles and purpose.
• Lead clients to identify needs and develop wellness plans.
• Review documentation and assess eligibility for services.
• Coordinate in-home supports and contracted services (lawn care, snow removal).
• Maintain OHATS, Data Basics and Excel records for clients.
• Prepare financial reporting for local site budgets.
