The Nurse Practitioner will work in collaboration with the Niagara Health Hospitalist program. This role is responsible for ongoing assessment and management and planning for clients deemed appropriate/referred by Hospitalists initially at the Welland Site on the in-patient units, rotating to other Niagara Health sites as required. The Nurse Practitioner is responsible for ongoing assessment, planning, management and treatment of patients within the complex care unit in collaboration with the Physicians. The Nurse Practitioner will report directly to the Site/Program Director. The Nurse Practitioner will provide leadership and promote excellence in patient-centered care through expert practice, education, consultation and research. The successful candidate will provide care with an emphasis on wellness, prevention and disease management through direct patient care, health promotion and education, outreach, advocacy and community development as required. This role will entail working with acute and post acute patient populations.
