PERSOL in Brazil’s Norte region seeks a dedicated Care Coordination Lead to oversee home care services. You will manage coordinators, drive client onboarding, and educate caregivers to ensure timely, high-quality care delivery.

Travel to client homes is required to supervise staff, assess needs, and develop care plans while building partnerships with community services to support patient-centered care.

Qualificações

• Bachelor's Degree or Diploma in Health Science Management, Social Services, or related discipline.
• Minimum of 3 years of relevant experience in healthcare or community care operations with strong people management skills.
• Demonstrated ability to engage effectively with internal and external stakeholders while delivering excellent customer service.
• Resourceful and knowledgeable about senior care needs and available support services.

Responsabilidades

• Oversee allocation and management of care coordinators for home care clients, ensuring timely and quality service delivery.
• Engage with clients and their families to facilitate the onboarding process and support care requirements.
• Assess and educate caregivers on best practices for client support and care planning.
• Conduct regular home visits to strengthen relationships and enhance client engagement.
• Monitor manpower deployment to ensure seamless operations and service continuity.
• Facilitate recruitment and onboarding of care staff, ensuring a smooth transition into the role.
• Provide ongoing supervision, coaching, and performance monitoring of care staff.
• Perform assessments and develop care plans to address health and social vulnerabilities of clients.
• Conduct scheduled re-assessments to track client progress and adjust care plans accordingly.
• Coordinate service delivery and follow-up actions to ensure continuity and quality of care.
• Establish and nurture community partnerships to enhance client service offerings.
• Monitor quality improvement initiatives and propose strategies for enhancing care processes.
• Support operational developments, including new service pilots and workflow enhancements.
• Provide secretariat assistance for meetings and fulfill additional duties as assigned.
