Job summary

DCC Industry Group is seeking a T2A Coordinator to assist young people aged 15 to 18 as they move toward adulthood and independence in Queensland. You will work across residential, foster and kinship care, and disability services to ensure transition planning starts early and leads to practical outcomes.

You will guide readiness assessments, keep the young person’s views central, and coordinate with care teams and external services to turn goals into tangible support.

Qualifications

• Bachelor's degree in social work, human services, psychology, or related field; diploma with sector experience may suffice.
• At least three years’ experience in residential care, transition to adulthood, kinship or disability services.
• Valid driver’s licence and ability to travel between locations.
• Current Working with Children Check, NDIS Worker Screening, First Aid/CPR, national police check.

Responsibilities

• Guide transition readiness assessments across life skills, housing, budgeting, health, identity, education and work.
• Maintain young people’s views, culture and goals in transition plans.
• Help care teams turn goals into practical support and opportunities for independence.
• Coordinate with Child Safety, post-care, housing, health and NDIS providers for referrals and follow-up.
• Support young people to maintain connections with family and community.
• Identify gaps and escalate risks when needed.
• Maintain accurate records and monitor progress as young people approach 18.
