Job summary

Allambi Care in Victoria seeks a TCP Key Worker to support young people to transition into suitable care arrangements, including independent living, foster care, or reunification with families. The role entails ongoing case management of funded packages and collaborating with families and service partners.

Applicants should have a Diploma in Community Services or equivalent, with experience in case work with children and young people, leadership ability, and strong written and verbal

Qualifications

• Diploma or equivalent in community services required.
• Experience in case work with children and young people.
• Leadership experience with employees.
• Knowledge of Needs Based Framework and TCI concepts.
• Current Working with Children Check and Police Checks required.
• Valid Driver’s Licence.

Responsibilities

• Provide support to young people to transition into suitable care arrangements.
• Manage and implement TCP funded packages and overarching case management.
• Collaborate with families, carers and service partners.
• Maintain accurate and confidential client files per policy.
