Job summary

Responsibilities

• Inform DCP Case Managers about placement program referrals and criteria.
• Achieve placement program KPI’s through partnerships with providers.
• Develop systems to enhance placement program referrals.
• Identify suitable placement destinations for Next Steps and SPC.
