Job summary

Department of Education in Western Australia is seeking to establish a pool of Youth Transition Coordinators to service the East Kimberley region. Youth Transition Coordinators are responsible for providing individualised case coordination and support services for students aged 10 to 18 years old who are exiting custodial sentences, transitioning to education, training and employment options.

Youth Transition Coordinators should work within relevant acts and maintain strong professional networks

Qualifications

• Experience working with youth and families in education or community settings.
• Ability to work autonomously within a multidisciplinary team.
• Strong communication and interpersonal skills for diverse communities.

Responsibilities

• Provide individualized case coordination and support for students aged 10–18 exiting custodial settings.
• Connect youth to education, training, employment and counselling services.
• Collaborate with schools, community partners and attendance/engagement leadership.
• Participate in planning and travel as required within the East Kimberley region.
