Job summary

Life Without Barriers is seeking a Case Manager – Next Step Plus for the North Coast region. You will support young people aging out of care, delivering outreach-based case management and tailored plans to help them live independently and achieve goals.

You’ll work across housing, education, employment, health and wellbeing, coordinating services and advocating with families, carers and community partners to secure sustainable outcomes.

Qualifications

• Experience providing case management to young people with complex needs.
• Excellent communication and documentation skills.
• Ability to work independently and manage competing priorities.
• Trauma-informed and strengths-based practice understanding.

Responsibilities

• Manage caseload with outreach-based, strengths-focused support.
• Assist housing, education, employment, health and wellbeing goals.
• Coordinate services and advocate for young people with stakeholders.
• Travel across the service area to deliver in-person support.
