Lifestyle Solutions is seeking a dedicated Case Worker for the Tamworth cohort. You will translate case plans into hands-on support, working directly with care teams in homes and with young people to create stability and positive long-term outcomes.
Role combines case management with frontline practice, supporting up to six children and young people, with paid training, career growth, and salary packaging benefits.
Help turn case plans into real outcomes for young people.
If you’re passionate about supporting children and young people in Out-of-Home Care and want a Case Worker role where you can see the impact of your work firsthand, this could be the opportunity for you.
At Lifestyle Solutions, our Case Workers don’t just develop plans from behind a desk. You’ll work directly alongside our care teams in the homes, helping translate therapeutic and case plan goals into consistent, meaningful everyday support.
With a caseload of up to six children and young people, you’ll have the opportunity to build strong relationships and help create stability, connection and positive long‑term outcomes.
Your week combines approximately 20 hours of case work with 18 hours working alongside frontline teams in the homes, helping turn plans and therapeutic strategies into everyday practice.
Lifestyle Solutions supports children and young people through therapeutic, evidence‑based care.
Guided by our values — Inspired, Courageous, Skilful and Responsive — we’re committed to creating safe, supportive environments where children and young people can build connections, develop confidence and work towards positive futures.
If you’re an experienced Case Worker — or have strong experience supporting children and young people and are ready to take the next step — we’d love to hear from you.
