Job summary

Life Without Barriers is seeking a dedicated Case Manager – Family Based Care to support children and young people in Out of Home Care. You will coordinate care, safety and wellbeing, review plans, and advocate for the child’s best interests.

Join a team that collaborates with families, carers and government agencies to achieve positive outcomes and permanency planning where appropriate. The role involves travel to placements and on-call duties.

Qualifications

• Bachelor level qualifications in Social Work, Social Science, Psychology or Human Services.
• Knowledge of trauma-informed care and responses to pain-based behaviours.
• Experience providing case management to children and young people, preferably in child protection context.
• Ability to manage a caseload with varying complexity and priorities.
• Experience developing and implementing case/plans and reporting to a high standard.
• Ability to work with diverse individuals and groups respectfully.
• It is required to hold a current unrestricted driver's licence.

Responsibilities

About the role

This is a challenging and rewarding opportunity for you to make a positive change in the lives of children and young people in Out of Home Care.

As a case manager you will be responsible for coordinating and overseeing the care, safety, and wellbeing of children and young people who are unable to live with their birth families.

The role involves assessing needs, developing and reviewing case plans, supporting placement stability, facilitating access to education, health and therapeutic services, maintaining relationships with families and carers where appropriate, and advocating for the child’s best interests.

As a Case Manager you will work collaboratively with children, carers, families, government agencies and other professionals to achieve positive outcomes, ensure compliance with legislative and policy requirements, and support permanency planning, reunification, or transition to independent living when appropriate.

About the program

We are recruiting a Case Manager to join our Family Based Care Program. We work closely with the Department of Families, Fairness and Housing to provide care arrangements either through Foster Care or a Targeted Care Package (TCP).

As a program we are responsible for ensuring that any child or young person placed in our care is provided with a safe and nurturing environment. We work closely with DFFH and other stakeholders to ensure that we are supporting the child in the context of their case plan and court order. As a case manager, you will be responsible for ensuring that the program requirements for home-based care are implemented into practice.

Key Responsibilities

• Develop and maintain functional and supportive relationships with children/young people and their families to promote positive outcomes
• Providing case management to children and young people in a range of different out of home care arrangements, predominately Foster Care. Your caseload may also include supporting young people through a Targeted Care Package (TCP) where they could be living with parents, kinship carer or in independent living.
• Building positive relationships with children, young people and carers through regular home and outreach visits
• Ensuring that we are including the client voice in our care planning and preparing them for leaving care.
• Working as part of a care team to ensure the co-ordination of supports and services to ensure the child and young person’s health, development and wellbeing needs are being met.
• Providing regular support and coaching to foster carers to ensure they provide a safe and secure environment for the children and young people in their care.
• Support and monitor placements ensuring carers participate in case planning process
• Participate in on call roster as required

Skills & Experience

• Bachelor level qualifications in Social Work, Social Science, Psychology or Human Services
• Knowledge of trauma informed care and responses to pain based behaviours.
• Experience providing case management to children and young people, preferably within the child protection context.
• Demonstrated ability to manage a caseload ranging in complexity and able to prioritise competing demands.
• Experience developing quality case plans or individual plans as well as leading the implementation of the goals within the plan.
• Experience working with individuals, families or groups from a culturally appropriate perspective.
• Demonstrated ability to communicate effectively with a range of stakeholders and being able to produce written reports and case notes to high standard.
• It is a requirement that you hold a current unrestricted driver’slicence.

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
