Job summary

Life Without Barriers Toowoomba is seeking a Case Manager – Next Step Plus to support young people transitioning from out-of-home care. You will manage a caseload, deliver outreach-based, strengths-focused case management, and help with housing, education, employment, health and independent living.

This is a six-month maximum-term parental leave replacement with no extension. Based in Toowoomba, you’ll travel within the service area, work autonomously and collaborate with a supportive team.

Qualifications

• Experience providing case management to young people with complex needs.
• Strong assessment, planning, engagement and advocacy skills.
• Ability to coordinate services and build effective stakeholder relationships.
• Excellent communication, documentation and organisational skills.
• Understanding of trauma-informed and strengths-based practice.

Responsibilities

• Develop goal-focused case plans tailored to each young person.
• Support access to housing, education, employment, health and wellbeing services.
• Build independent living skills and strengthen family, cultural and community connections.
• Advocate for young people and coordinate services for sustainable outcomes.
• Collaborate with Child Safety, community organisations and other stakeholders.
