Job summary

CatholicCare NT in Darwin is seeking a dedicated case worker to provide intensive case management for youth aged 12–25 and their families. You will assist participants to attend appointments, offer practical support, and help run group work and information sessions.

The role emphasizes culturally sensitive practice, collaboration with stakeholders, and successful navigation of complex trauma and mental health concerns.

Qualifications

• Relevant qualification in community services, youth work, social work or similars.
• Strong interpersonal and communication skills including case note writing, time management, and the ability to liaise with stakeholders.
• Demonstrated cultural competency, particularly in working with Aboriginal and/or Torres Strait Islander People.
• Experience working with young people (12-25 years) with complex trauma, substance use and/or mental health challenges.
• Ability to support case management activities and assist with the facilitation of groups and information and education sessions.

Responsibilities

• Provide case management support for participants to achieve their goals, including coordinating appointments and practical support.
• Support group work and facilitate information and education sessions as required.
• Liaise with families/carers and other stakeholders to ensure coordinated care.
• Maintain accurate case notes and documentation in line with organisational policies.
