Job summary

CatholicCare NT is seeking a dedicated Case Manager to support young people aged 12–25 in Darwin and Palmerston through the DAISY and YES programs. You will provide intensive case management, practical assistance, and education sessions to help participants achieve their goals.

Qualifications

• Relevant qualification in community services, youth work, social work or similars.
• Strong interpersonal and communication skills including case note writing, time management, and liaising with stakeholders.
• Demonstrated cultural competency, particularly in working with Aboriginal and/or Torres Strait Islander People.
• Experience working with young people (12-25) with complex trauma, substance use and/or mental health challenges.
• Ability to support case management activities and facilitate groups and information sessions.

• Provide case management support to participants to achieve goals.
• Assist participants to attend appointments and provide practical support.
• Support group work and information/education sessions.
• Liaise with stakeholders to coordinate services.
