Job summary

Kanda is seeking compassionate, reliable workers to provide person-centred, trauma-informed support in home and community settings across Queensland. You will assist with personal care, daily routines, appointments and household tasks, while promoting participant rights, dignity and independence.

You'll work to support community access, social participation and employment goals, provide emotional support during distress, and follow individualised support and behaviour plans.

Qualifications

• 1+ years' experience in disability support, aged care or community services
• Experience supporting participants with complex mental health and psychosocial needs is desirable
• Strong documentation, reporting and incident-management skills
• Ability to work independently and mentor newer team members

Responsibilities

• Providing one-to-one support in the participant's home and community
• Supporting personal care, daily routines, appointments, and household tasks
• Assisting with community access, social participation and employment goals
• Providing reassurance and emotional support during periods of distress
• Supporting medication prompts and health routines
• Following individualised support and behaviour support plans
• Identifying and appropriately responding to changes in presentation
• Building positive, respectful relationships with the participant, their family and their support team
• Completing shift notes, progress notes and incident documentation
