Job summary

Sydney Local Health District is seeking a Social Worker to join the OOHC Health Pathway Program at Croydon Health Centre. The role centers on trauma-informed psychosocial assessment and Health Management Plans for children in Out of Home Care, aged 0–18, in collaboration with the OOHC Health Co-ordinator.

The position is temporary part-time (24 hours/week) with potential extension. Must hold a Social Work degree eligible for AASW and a NSW Driver Licence; Tuesday work required.

Qualifications

• Bachelor or Post Graduate degree in Social Work with eligibility for AASW.
• Current NSW Driver Licence (P1 and P2 acceptable).
• Advanced trauma-informed and complex psychosocial assessment knowledge.
• Experience in health care coordination and OOHC child protection context.

Responsibilities

• Facilitate multidisciplinary health reviews for OOHC program.
• Provide trauma-informed psychosocial screening and Health Management Plans.
• Adapt delivery mode according to need and resources.
