Sydney Local Health District is seeking a Social Worker Level 3 for the Out of Home Care Health Pathway Program based at Croydon Health Centre. The role involves coordinating multidisciplinary health reviews and developing Health Management Plans to address the health and wellbeing needs of children in OOHC.

The ideal candidate will hold a Social Work degree with AASW eligibility, possess trauma-informed assessment expertise, NSW Driver's Licence, and be able to work three days per week

Qualifications

• Bachelor or Post Graduate degree in Social Work with eligibility for full membership of the Australian Association of Social Workers (AASW).
• Current NSW Driver's Licence (P1 and P2 acceptable).
• Advanced skills in trauma-informed psychosocial assessment for children and young people in Out of Home Care.

Responsabilités

• Facilitate multidisciplinary health reviews with the OOHC Health Co-ordinator.
• Develop and implement Health Management Plans addressing physical, developmental, psychosocial, sexual and mental health needs.
• Deliver trauma-informed psychosocial screening and coordinate care within resource constraints.
• Adapt delivery mode to the needs of the program and clients.
