Job summary

NBMLHD, Nepean Blue Mountains Local Health District, is seeking an Aboriginal Health Worker to join the Out-of-Home Care Health Pathway Program. You will provide culturally responsive assessment, engagement and care coordination for children and young people in care, working with health professionals, carers and families to embed cultural identity and safety in health planning.

This is a temporary part-time role, identified, requiring Certificate III in Aboriginal Primary Health Care or

Qualifications

• Demonstrated understanding of Aboriginal culture and ability to apply culturally safe practice.
• Strong interpersonal and communication skills with children, families and carers.
• Willingness to undertake Certificate III in Aboriginal Primary Health Care within 12 months.
• Current driver’s licence and ability to travel across the LHD for work purposes.

Responsibilities

• Provide culturally responsive assessment, engagement and care coordination for Aboriginal children and young people in Out-of-Home Care.
• Collaborate with health professionals, carers and families to ensure cultural identity, safety and needs are embedded in care planning and service delivery.
• Advocate for access to health services and navigate a coordinated health pathway within the system.
