Job summary

Winnunga Nimmityjah Aboriginal Health and Community Services is seeking an experienced Case Manager to join the Male Residential Rehabilitation Service in Watson, ACT. You will provide strengths-based, trauma-informed case management to Aboriginal and Torres Strait Islander men, supporting recovery, wellbeing and transition back into community life.

You will develop individual case plans, support daily program activities, coordinate referrals and collaborate with stakeholders to achieve positive

Qualifications

• Minimum three years of case management experience in AOD or residential rehab.
• Understanding Aboriginal health and wellbeing, culture, and self-determination.
• Current Driver Licence and ability to obtain WWVP registration.

Responsibilities

• Provide strengths-based, trauma-informed case management to residents.
• Develop and implement individual case plans supporting recovery and wellbeing.
• Support residents with daily residential program and rehab activities.
• Coordinate referrals to health, housing, legal, family and community supports.
• Identify risks and respond to complex situations with appropriate actions.
• Maintain accurate case notes, assessments and documentation.
• Participate in reviews, care planning and handovers.
• Collaborate with internal and external stakeholders to achieve positive outcomes.
