Job summary

The GEO Group Australia is seeking a Throughcare Case Manager to support men transitioning from custody to the community. This role delivers pre-release planning and post-release support in a trauma-informed, strengths-based approach.

You will partner with internal and external stakeholders to design and deliver programs at the Community Reintegration Centre, contributing to safer, more successful reintegration outcomes.

Qualifications

• Relevant tertiary qualification or equivalent experience.
• Experience in trauma-informed care.
• Experience with case management and community reintegration.
• Ability to build stakeholder relationships.
• Experience with motivational interviewing.

Responsibilities

• Maintain a caseload of clients with reintegration needs and provide assertive outreach and support.
• Provide effective case management and coordination for post-release plans.
• Coordinate delivery of programmes and services to meet reintegration needs.
• Assist with housing, education, employment, and health support post-release.
• Maintain accurate attendance, case notes and progress records.
