The Institute for Urban Indigenous Health Ltd (IUIH) is seeking a Team Leader Aged Care in Margate, Queensland. You will manage client contracts, coordinate a team of care workers, and ensure services meet client care plans and approved standards. Leadership and community collaboration are central to the role.

You will work to maintain quality, timeliness and effectiveness of service delivery, liaising with assessors, nurses and other providers to support Indigenous elders in their own homes.

Qualifications

• Current Criminal History Check.
• Current C Class Drivers Licence (QLD).
• Current Disability Worker Screening Check (Qld Yellow Card).
• Current First Aid and CPR Certificates.

Responsabilités

• Demonstrate practical and innovative leadership in the design implementation and evaluation of daily programs for clients.
• Liaise with the Assessor and Community Nurses to ascertain Client needs.
• Identify a range of services that enhance inclusion for elderly Indigenous persons who are residing in their own homes within the community.
• Collaborate with other service providers to ensure effective service delivery and increase community networks.
• Coordinate and review service arrangements with Service Providers.
