Job summary

IUIH is seeking a Care Coordinator for MATSICHS Social Health to provide coordination of supports to Aboriginal and Torres Strait Islander clients facing complex social and mental health challenges. You will undertake structured intake, screening, and triage, deliver brief psychosocial support within scope, and collaborate with clinical staff on higher-level assessment.

• Certificate or Diploma in Community Services, Mental Health or similar.
• Current Working with Children's Check (Qld Blue Card) or AHPRA Registration.
• Current Criminal History Check.

Responsibilities

• Act as central intake point for Social Health referrals.
• Conduct initial intake, screening and structured assessments (non-clinical).
• Escalate high-risk presentations to Clinical Care Coordinators or senior clinicians.
• Support development of care plans and coordination of services.
• Manage a caseload with follow-up and engagement.
• Work across Caboolture, Margate and Strathpine as needed.

Education
