Job summary

UnitingCare Queensland FaDS is seeking a casual Supported Living Facilitator in Oxley to support individuals in daily living, personal care and independence goals. You will work with the person, their family and support networks to help realise personal aspirations with emphasis on choice and dignity.

You’ll contribute to developing and reviewing individual plans, enabling autonomy over services while maintaining respect for privacy and safety.

Qualifications

• A current NDIS Worker Screening check.
• Open Australian Driver's Licence.
• CPR and First Aid certificates current.
• Ability to communicate in spoken and written English.

Responsibilities

• Work with the person supported, family and networks to achieve life goals.
• Provide daily living support including personal care with emphasis on choice.
• Develop, implement and review individual plans focusing on life goals.
• Empower autonomy over services and life decisions.
• Respect privacy, dignity and confidentiality of the person and family.
