Job summary

UnitingCare Queensland is seeking a Supported Living Facilitator to assist clients in the community, promoting independence and well‑being in Plainland. You will visit clients at home, provide daily living support, and help with transportation and appointments on a casual basis.

CPR and First Aid are required before starting, and a valid driver’s license with vehicle access is essential. You will work mainly Monday to Friday 9am–5pm, with travel to nearby towns as needed, and enjoy flexible work

Qualifications

• Ability to communicate effectively in spoken and written English.
• Willingness to travel between client locations in the community.
• Positive attitude and willingness to learn in a disability/aged care setting.

Responsibilities

• Provide in-home personal care and daily living assistance per Care Plan.
• Travel between clients using own vehicle and support with appointments.
• Support community-based recreational activities and transportation needs.
