Job summary

UnitingCare Queensland is seeking a Supported Living Facilitator to support people with disability in daily living, personal care and goal planning. This casual role requires flexibility and strong communication, with NDIS pathways and eligibility supported by our FaDS team.

You will work closely with individuals, families and networks to empower independence, dignity and choice, contributing to a compassionate, inclusive service across Queensland.

Qualifications

• Experience in disability services and supporting people with autonomy.
• Current NDIS Worker Screening check and Blue Card; assistance available to apply.
