Job summary

UnitingCare Queensland FaDS is seeking a Supported Living Facilitator to support people in daily living, focusing on choice and quality of life in Warana and Palmview. You will work with individuals, families and informal networks to help achieve hopes and aspirations.

This casual role offers flexible hours, on-the-job development and a strong focus on safety, respect and dignity. You’ll contribute to individual plans, promote autonomy and maintain confidentiality.

Qualifications

• Experience in disability, aged or community sector is ideal but not essential.
• NDIS Worker Screening check and Blue Card required or ability to obtain prior to employment.
• Current CPR and First Aid Certificate required before starting.
• Excellent spoken and written English communication skills.
• Reliable, flexible with varied hours and days; team-oriented.

Responsibilities

• Work with the person supported, family and networks to help them achieve their life goals.
• Provide support in everyday living, including personal care, focusing on choice and quality of life.
• Contribute to development, implementation and review of individual plans.
• Enable autonomy in daily services while respecting privacy and dignity.
• Uphold rights to respect, privacy, confidentiality for the person and family.
