Job summary

UnitingCare Queensland is looking for a Casual Supported Living Facilitator in Disability Services (NDIS) to support people in their everyday living. The role offers $45.28 p/hr, Super, and NFP Salary Packaging, with applications reviewed as received.

You will work with the person supported, their family and networks to help achieve life goals, provide personal care, and participate in developing individual plans. A positive, flexible, team-oriented environment is described.

Qualifications

• Experience in Disability or community services preferred but not essential.
• Current CPR and First Aid certificates.
• NDIS Worker Screening check and Blue Card (assistance to apply if needed).

Responsibilities

• Work in partnership with the person being supported, their family and informal support networks to attain their hopes and aspirations.
• Provide support to people in everyday living situations, including personal care as identified, with emphasis on choice and quality of life.
• Participate in development, implementation and review of individual plans focusing on life goals and autonomy.
• Enable people to attain personal autonomy over their services.
• Uphold the right to privacy, dignity and confidentiality of the person and their family.
