Job summary

UnitingCare Queensland is seeking a Supported Living Facilitator to provide in-home care to clients in the Caloundra area. You will help with daily living activities, mobility, and community participation, visiting clients at their homes on a schedule.

The role requires a current NDIS screening, CPR/First Aid, a valid open driver’s license, and access to a vehicle. Flexible work patterns and non-profit salary packaging options are available.

Qualifications

• Current NDIS Worker screening check.
• CPR and First Aid certificate.
• Open Driver's License and access to a vehicle.
• Car Insurance in your name.
• Must be available to drive between clients with your own vehicle.
• Hours are mainly Monday to Friday 9am–5pm.
• Experience in Disability/Aged/Community sector is ideal but not essential.
• Able to communicate in spoken and written English.

Responsibilities

• Provide in-home personal care and daily living assistance.
• Drive between clients using own vehicle.
• Complete scheduled visits and interact with clients and families.
• Assist with community-based recreational activities and appointments.
