Job summary

UnitingCare Queensland FaDS team is seeking a Supported Living Facilitator to deliver in-home care services to clients in the Caloundra area on a casual basis. You will travel between client homes, assist with daily living tasks, and support community activities as outlined in each Care Plan.

This role requires driving, a current NDIS screening, and CPR/First Aid certificates. You will contribute to improving clients’ independence while working with a supportive, not-for-profit organisation.

Qualifications

• Experience providing in-home care and daily living assistance.
• Strong spoken and written English communication skills.
• Current NDIS Worker screening check is required.
• CPR and First Aid certificates are required before you start.

Responsibilities

• Provide in-home personal care and daily living assistance to clients.
• Travel between clients using your own vehicle.
• Follow individual Care Plans and support community activities.
• Assist with transportation to appointments and shopping.
