Job summary

Yapi Services is seeking a Support Worker/Mentor to provide individualized support and mentoring to participants across community, home and other settings. You will help participants achieve personal goals, increase independence, participate in the community and develop positive connections while upholding dignity, privacy and professional boundaries.

Qualifications

• NDIS Worker Screening Check
• Working with Children Check
• Working with Vulnerable People Check
• Police Clearance
• Current First Aid & CPR certification
• RRHAN-EC training
• Current driver's licence
• Access to a reliable, registered vehicle
• Strong interest in helping people reach goals
• Confident in building relationships with children, young people and adults
• Strong communication and interpersonal skills

Responsibilities

• Provide culturally safe, respectful and person-centred support and mentoring.
• Build positive, professional and trusting relationships with participants.
• Support participants to work towards their individual goals and aspirations.
• Encourage independence, self-determination, confidence and decision-making.
• Support community access, social participation and connection to community.
• Assist with appropriate daily living activities and practical tasks.
• Provide transport and support participants to attend appointments, activities, education, employment and community events where required.
• Support participants to develop life skills, social skills and positive routines.
• Maintain professional boundaries and uphold participant dignity, privacy and confidentiality.
• Complete clear, accurate and timely support notes, incident reports and other required documentation.

Job description

The Support Worker/Mentor will provide individualised support and mentoring to participants across community, home and other appropriate settings.
