Accordwest is seeking a dedicated Support Worker to join our Homelessness Services team. You will work within a multidisciplinary, trauma-informed framework to provide timely referrals, resources, and advocacy for clients awaiting case management.

You will stay organised in a fast-paced environment, collaborate closely with Case Workers, and contribute to building a safe, inclusive community. A WA driver’s licence and First Aid are required.

Formación

• Certificate IV or Diploma in a human service focused discipline, or substantial case management experience.
• Demonstrated experience supporting people with complex needs in a community services setting.
• Effective verbal, written and interpersonal communication skills.
• Current WA Driver’s Licence and First Aid Certificate.
• Ability to obtain Working with Children Check and Police Clearance.

Responsabilidades

• Manage incoming referrals and conduct initial assessments.
• Provide triage and support while clients await case management.
• Monitor client vulnerability and escalate concerns as needed.
• Maintain accurate data in client management systems and support reporting.
• Coordinate resources (e.g., welcome packs) and collaborate with Case Workers.
• Support team meetings and share knowledge across the program.
