Job summary

Accordwest is seeking a dedicated Support Worker to join the Homelessness Services team. You will support people experiencing homelessness with trauma-informed, culturally safe, and person-centred practice, coordinating referrals and resources as intake waits for case management.

You will work within a multidisciplinary team to ensure timely support, advocacy, and high-quality service delivery, while upholding our values of integrity, courage and collaboration.

Qualifications

• Effective verbal, written and interpersonal communication skills.
• Demonstrated experience in case management or similar community services role.
• Ability to obtain Working with Children Check (WA), National Police Clearance, Form 395 Department of Communities clearance.
• Current WA Driver’s Licence and First Aid Certificate.

Responsibilities

• Manage incoming referrals and conduct initial assessments; provide triage support for clients awaiting case management.
• Maintain accurate, timely data in the client management system; support reporting and compliance with leadership.
• Prepare client resources (welcome packs) and manage inventory; coordinate with Case Workers on resource needs.
• Support the coordination and documentation of team meetings.
• Collaborate with Case Workers and team members to share knowledge and develop internal support networks.
