Job summary

Claro Disability Services is looking for caring professionals to support clients with complex needs across mobility, daily activities, and medical supports. You will work closely with families and medical teams to tailor plans that enhance independence and participation in meaningful activities.

You’ll be part of a values-driven team, delivering person-centered care in a flexible schedule environment, with ongoing clinical support and opportunities to grow in the disability services field.

Responsibilities

• Person-centered planning: chat with client and family/advocates to understand goals and routines.
• Complex physical care: assist with mobility, transfers, positioning and daily tasks as outlined in the plan.
• Medical supports: monitor changes, follow plans, liaise with care team.
• Social time and participation: bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: support decision-making and consent, boost independence where possible.
• Cultural humility: respect backgrounds, beliefs, and ways of being.

What you’ll do (*tailored to each client as required*)

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
