Job summary

Claro Disability Services in Australia provides person-centred supports to people with complex needs, helping them live independently with dignity. Qualifications

Responsibilities

• Conduct person-centered planning with client and family to understand goals and routines.
• Provide complex physical care: mobility, transfers, positioning, daily tasks and devices.
• Monitor medical needs, follow plans and liaise with care team.
• Support social time, participation and community access aligned with interests.
• Promote autonomy, advocate, respect consent and independence.
• Respect cultural backgrounds and values in all interactions.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
