Claro Disability Services in Melbourne is seeking a dedicated care assistant to support people with complex needs. You’ll work closely with clients, families, and medical teams to implement person-centered plans, assist with mobility and daily tasks, monitor health needs, and enable meaningful social participation.

Responsabilidades

• Person-centered planning: chat with client and family/advocates to understand goals.
• Complex physical care: mobility, transfers, positioning, daily tasks, devices.
• Medical supports: monitor changes, follow plans, liaise with care team.
• Social time and participation: provide meaningful activities aligned with interests.
• Autonomy and advocacy: support decision-making and consent.
• Cultural humility: respect backgrounds and beliefs.
• Mealtime management, manual handling, medication administration, PEG desirable, changed behaviour.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
• Mealtime Management, Manual Handling, Medication Administration, PEG desirable, Changed Behaviour

Week 2

• Mon 1600 - 0900
• Tues 2300 - 0700
