Job summary

Claro Disability Services is expanding its disability supports team in Australia. We offer person‑centred planning, companionship, and mobility assistance that empower clients to live more independently.

You’ll work closely with families and clinical staff to tailor goals, routines and care plans, delivering compassionate, adaptable care in a wheelchair and mobility‑assisted environment.

Responsibilities

• Provide person‑centred planning and engage with clients and families to understand goals and routines.
• Assist with mobility, transfers and daily tasks as outlined in care plans.
• Support medical needs and liaise with clinical teams when plans change.
• Enable social participation and community access aligned with client interests.
• Support autonomy, consent and advocacy while respecting cultural backgrounds.
• Collaborate with families, professionals and other support workers to ensure smooth delivery of care.

What you’ll do (*tailored to each client as required*)

• Person-centered planning: Chat with the client (and family/advocates), get to know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision‑making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being

Who we’re looking for

• Reliable and professional, with good notetaking and teamwork.
