Job summary

Claro Disability Services in Nunawading, Melbourne is seeking compassionate support workers to assist people with complex needs in living more independent lives. You will tailor supports to each client's goals, routines, and cultural values, while working closely with families, medical professionals, and other carers.

Qualifications

• Person-centered planning experience.

• Competent in accurate note-taking and documentation.

Responsibilities

• Engage in person-centered planning with client and family/advocates.
• Assist with mobility, transfers, positioning and daily tasks as per plan.
• Support medical needs, monitor changes and liaise with care team.
• Facilitate social time and community participation according to client interests.
• Support autonomy, respect consent and promote independence where possible.
• Respect and incorporate client cultural backgrounds and values.

What you’ll do (*tailored to each client as required*)

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.


• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan


• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.


• Social time and participation: Bring meaning, fun, and community access aligned with interests.


• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.


• Cultural humility: Respect backgrounds, beliefs, and ways of being
