Claro Disability Services seeks dedicated disability support workers to provide person-centered care for clients with complex needs across shifts. You’ll support mobility, daily activities, and health plans while fostering autonomy and community access. Collaboration with families, medical teams, and other support staff is essential.

• Provide person-centered support tailored to each client’s goals and routines.
• Assist with mobility, transfers, positioning and daily tasks as outlined in care plans.
• Support medical and clinical plans and liaise with care teams.
• Support social participation, autonomy, and advocacy for clients.
• Respect diverse backgrounds and cultures in care delivery.

What you’ll do (*tailored to each client as required*)

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being

• Interest or experience in complex care, disability, and health nuances.
• Great communicator: listens well, reads cues, and adapts to different styles.
• Reliable and professional, with good notetaking and teamwork.
• Flexible and adaptable; comfortable with changing needs and shifts.

• Optional but nice: Manual handling, epilepsy management, or disability-support training.
