Job summary

Claro Aged Care And Disability Services is seeking a dedicated Support Worker to assist clients in accommodation settings. You’ll tailor supports to individual goals, coordinate with families and health professionals, and contribute to a respectful, person-centered care plan.

Shifts are varied, requiring empathy, reliability, and strong teamwork to create meaningful daily routines and promote independence for clients with complex needs.

Qualifications

• A person-centered, warm, patient, and genuinely caring soul.
• Interest or experience in complex care, disability, and health nuances.
• Great communicator: listens well, reads cues, and adapts to different styles.
• Reliable and professional, with good notetaking and teamwork.
• Flexible and adaptable; comfortable with changing needs and shifts.
• Optional but nice: Manual handling, epilepsy management, or disability-support training.

Responsibilities

• Chat with the client (and family/advocates) to understand goals and routines.
• Assist with mobility, transfers, daily tasks, and devices as outlined in the plan.
• Monitor medical plans, liaise with the care team, and respond to changes.
• Provide social time and community access aligned with interests.
• Support decision-making, respect consent, and promote independence.
• Respect diverse backgrounds, beliefs, and cultures.

What you’ll do (*tailored to each client as required*)

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
