Responsibilities

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
• You will be working with clients with stroke, Spina Bifida, Non verbal, complex care ,Stoma care, bladder care, catheter care, bowel care ,meal prep, personal care, medication, community participation, use of electric wheelchair, cleaning, cooking, shopping, client specific management plans.

• Tues - 7.30am to 1.30pm
• Wed - 7.30am to 1.30pm
• Sat - 2.30pm to 8pm
• Sun - 6pm to 7am inc S/O (11pm - 7am)
