Responsibilities

• Person-centered planning: chat with client and family to understand goals and routines.
• Complex physical care: mobility, transfers, positioning, daily tasks, devices per plan.
• Medical supports: monitor changes, follow plans, liaise with care team.
• Social time and participation: create meaningful activities and community access.
• Autonomy and advocacy: support decision-making and consent; promote independence.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
