Beacon Support is seeking a dedicated Support Worker to deliver client-focused services under a client-specific Support Plan aligned with NDIS standards. You will provide activities of daily living, companionship and capacity-building support, while coordinating with the Care Services Coordinator and other stakeholders.

Support Workers are responsible for the day-to-day delivery of services and supports to clients of Beacon Support. The delivery of these services and supports must be individually focused and delivered in accordance with the client's Support Plan, and align with The Beacon Way, Beacon's policies and procedures, and industry standards including the NDIS Code of Conduct. The role of Support Worker includes providing active support and companionship, assessing and implementing capacity building strategies aligned with clients' NDIS goals. Where required, a worker may be identified as the Key Support Worker for a client and undertake coordination tasks, liaising with internal and external stakeholders to assist in facilitating care requirements under the direction of a Care Services Co-ordinator.
Key responsibilities
Provide activities of daily living for clients in line with their support requirements, including behavioural support, personal care and hygiene, home duties, fitting and removal of aids and appliances, monitoring medications, shopping, meal planning and preparation, organising appointments, assistance with care of pets, gardening, vocational and leisure activities, budgeting, transport, and training
Actively engage with clients when undertaking supports and provide companionship within the scope of the role, maintaining appropriate and professional boundaries
Promote positive daily routines and structure which are meaningful and purposeful to the individual, with the aim of building their capacity, confidence, and independence
Ensure that supports provided to clients are consistent with Beacon Support's policies, processes, and guidelines, all of which are based on a framework of Positive Behaviour Support
Provide specialised care to clients with particular emphasis on those requiring extra or complex supports due to specific physical disabilities or frailty
Assist clients to develop a greater awareness of their strengths, needs, and support strategies
Assist clients to work towards their goals and provide the supports required to achieve them
Support community access activities including accompanying clients on social outings, appointments, and/or shopping
Maintain appropriate client records utilising Beacon Support's documentation and reporting procedures
Report incidents, accidents, risks, complaints, and feedback on behalf of self, colleagues, and clients as required in a timely manner via internal systems
About you
Minimum Certificate III in Disability Support, Aged Care, Individual Support, or an appropriately related field (preferred)
More than six months' experience working in disability, aged care, or a related field is required if the Support Worker is unqualified. For qualified Support Workers, this experience is preferred
Knowledge of, or ability to rapidly gain understanding of, the National Disability Insurance Scheme (NDIS) and other relevant third-party schemes, and the care needs of people living with disability, aged care, and/or child safety
Previous experience in a similar role in a community services, aged care, and/or disability services environment
Proven ability to provide supports for people with complex health and/or disability needs in accordance with their Support Plan
Ability to use initiative and work independently or as directed
Ability to handle stressful and adverse situations with composure
Sound written and verbal communication skills and the ability to liaise with internal and external stakeholders
Current First Aid Certificate and annual CPR
Positive Notice Blue Card (Paid Employees), NDIS Worker Screening Clearance, current Queensland Driver's Licence, motor vehicle with third party and comprehensive insurance, and NDIS Modules
