Job description

Immediate Care is seeking a reliable, respectful and experienced Disability Support Worker to provide individualised support to an NDIS participant living in Ararat. The participant is an adult with an above-knee amputation who requires assistance with daily living, mobility and community participation. We are looking for a worker who can build a positive and professional working relationship while supporting the participant to maintain independence, choice and control.

Key responsibilities

• Assistance with daily living activities and routines
• Mobility support and assistance associated with physical disability
• Supporting safe access within the home and community
• Community access, appointments, shopping and social activities
• Assistance with household tasks where required
• Encouraging independence and supporting the participant to work towards their NDIS goals
• Providing respectful, person-centred support
• Maintaining appropriate progress notes and documentation
• Communicating with the participant and relevant members of their support team
• Following the participant's support plan, risk assessments and Immediate Care policies and procedures
