Job summary

Easy iCare PTY PLD in Melbourne seeks a compassionate Disability Support Worker to provide person-centred care at a participant's home and within the community.

You will assist with personal care, community access, and daily living tasks, follow positive behaviour strategies, complete shift notes, and collaborate with families and professionals to promote independence and safety.

• NDIS Worker Screening Check is required.
• Working with Children Check is required.
• First Aid Certification is required.
• CPR Certification is required.

Responsibilities

• Provide person-centred support tailored to the participant's needs and goals.

• Respond calmly to behaviours of concern and follow plans.
• Follow Positive Behaviour Support Plans and established strategies.
• Assist with household tasks and daily routines as required.

• Complete accurate shift notes and incident reports.

You will play an important role in promoting the participant's independence, dignity, safety, and quality of life.

Key responsibilities

Provide person-centred support tailored to the participant's individual needs and goals.

Assist with personal care, including showering, dressing, grooming, and other daily living activities.

Support the participant with community access, appointments, shopping, social activities, and recreational activities.

Provide support to individuals who may display behaviours of concern and respond calmly and appropriately to challenging situations.

Follow Positive Behaviour Support Plans and established support strategies.

Assist with household tasks and daily routines as required.

Maintain a safe, respectful, and supportive environment.

Encourage independence and support the participant to make informed choices.

Complete accurate shift notes and incident reports as required.

Work collaboratively with the participant, family, support team, and other professionals.
