Job summary

Empowering You Disability Services in Hervey Bay is seeking an experienced Support Coordinator to help NDIS participants implement plans and access vital services across the Fraser Coast region. You will build relationships with participants, families and providers, advocate for choices, and coordinate supports to deliver outcomes that matter to each person.

Qualifications

  • Experience in NDIS Support Coordination (preferred).
  • Sound understanding of the NDIS and relevant legislation.
  • Excellent written and verbal communication skills.
  • Strong computer literacy and ability to manage documentation.
  • Current Queensland Driver Licence.
  • Reliable motor vehicle with comprehensive insurance.
  • Current NDIS Worker Screening Clearance.
  • Current Blue Card (or ability to obtain).
  • Current First Aid & CPR Certificate.
  • Full Australian working rights.

Responsibilities

  • Coordinate and implement participants’ NDIS plans.
  • Assist participants to build independence and strengthen informal and community supports.
  • Connect participants with quality service providers and mainstream services.
  • Monitor service delivery and ensure supports remain aligned with participant goals.
  • Identify and address barriers impacting participants’ ability to access supports.
  • Maintain accurate and compliant case notes and documentation.
  • Prepare reports for the NDIA where required.
  • Liaise with allied health professionals, hospitals, guardians, families and other stakeholders.
  • Participate in case reviews and multidisciplinary meetings.
  • Ensure all services are delivered in accordance with the NDIS Practice Standards and Code of Conduct.

Coordination Network, a branch of Empowering You Disability Services (EYDS), is a growing registered NDIS provider committed to empowering people with disability to live the life they choose. We’re looking for an experienced and motivated Support Coordinator to join our team servicing the Fraser Coast region. If you’re passionate about helping participants build capacity, navigate the NDIS, and achieve meaningful outcomes, we’d love to hear from you.

About the Role

As a Support Coordinator, you will work closely with NDIS participants, their families, carers and support networks to implement NDIS plans and connect participants with the services they need to achieve their goals.
