Job summary

Future Focus Support Coordination seeks an experienced NDIS Support Coordinator to join the Fraser Coast team. This part‑time role offers flexible hybrid work, autonomy, and a collaborative environment focused on improving outcomes for participants.

You will support participants to implement plans, build networks, and access services across disability, health, housing, and community systems, while respecting participant choice and dignity.

About the Role

Working across the Fraser Coast region, you'll support NDIS participants to:

• Implement and maximise their NDIS plans
• Build strong support networks
• Navigate disability, health, housing, mental health, justice and community systems
• Strengthen their independence and decision-making
• Access the right services at the right time

This is a hybrid role with flexibility to manage your own diary, work from home, and conduct participant meetings across the region.

You’ll be supported by experienced leaders, regular supervision, and a collaborative team environment that understands the realities of Support Coordination.

• Has previous experience as an NDIS Support Coordinator
• Is positive, adaptable and solutions-focused
• Remains calm and resilient when supporting complex situations
• Enjoys working independently while still being part of a team
• Builds strong relationships with participants, families and providers
• Understands the importance of participant choice, control and dignity
• Has strong written and verbal communication skills
• Lives locally within the Fraser Coast region

Essential Requirements

• Previous Support Coordination experience
• Relevant qualification in Community Services, Social Work, Psychology, Mental Health, Allied Health or a related field (or substantial industry experience)
• Current Driver Licence
• Reliable vehicle
• NDIS Worker Screening Check
• Working with Children Check (or ability to obtain)
• Ability to satisfy relevant probity requirements
