Job summary

Alpine Health in Victoria is seeking a Manager - Support at Home to lead care coordination and management across the CHSP & HACC programs. This role combines operational leadership with workforce supervision of Intake Services, Care Partners and Service Coordinators.

The position requires tertiary Allied Health qualifications and demonstrated leadership in health and community services, with a focus on quality and person-centred care delivery.

Qualifications

• Tertiary qualification in Allied Health or Clinical field.
• Experience leading health/community services teams.
• Experience supervising staff and managing teams.

Responsibilities

• Lead and supervise the Intake Officers, Care Partners and Service Coordinators.
• Oversee assessment and service coordination within CHSP & HACC to meet targets.
• Manage staff rosters effectively.
• Drive recruitment across Support at Home, CHSP & HACC as required.
• Lead service improvement to enhance care quality and consistency.
• Foster a team culture with emotional intelligence.
• Ensure high-quality, person-centred care for participants.
• Support staff in promoting client independence and tailored care planning.
• Maintain strong relationships with clients, staff and stakeholders for person-directed care.
• Oversee implementation of care plans across programs.
• Use data to monitor performance, identify trends and drive improvements.
• Ensure regulatory and accreditation standards compliance.
• Lead internal QA activities and accreditation processes.
• Promote continuous improvement within the team.
• Ensure staff training and up-to-date knowledge.
• Understand KPI management and performance.
• Model professional communication and ongoing development.
